Studies have revealed that, whereas many prostatic calculi are formed as a result of concretions of prostatic gland secretions, many are composed of similar constituents to those of renal, ureteric and bladder calculi (urinary constituents), thereby confirming the tendency of urine to enter the prostate. AN ILLUSTRATED POCKETBOOK OF PROSTATIC DISEASES 33 Figure 28 Prostatic calculi can be seen on a plain X-ray, perhaps secondary to intrapro-static urinary reflux. Calculi tend to form within the prostate, where they may constitute a nidus for chronic prostatic infection ' Figure 29 Prostatitis is characterized by an inflammatory response within the gland.

A clear-celled ‘hypernephroid’ form is also described in this grading. Gleason grade 5 is characterized by sheets of malignant cells with little evidence of gland formation. Single-celled infiltration is common. Gleason scores 2–4 are well differentiated, scores 5–7 are moderately well differentiated and scores 8–10 are poorly differentiated varieties of prostate adenocarcinoma. These tumor gradings appear to correlate well with subsequent metastatic potential and overall survival. Cancers originating in the transitional zone tend to have a lower Gleason grade than tumors of the peripheral zone, an observation that appears to correlate well with the higher proliferative rates seen in the latter (Figure 36).

Devised during the Veterans Administration Cooperative Urological Research Group studies (1960–1975), different tumor patterns were identified without preconceived rating. Their presence was recorded and subsequently correlated with survival data by the study statistician.
